The application form is a Word document. You can fill it in and email it to Redkite or print it out, fill it in by hand and fax or post it. Redkite understands you may need someone to scribe for you, however the application should be in your own words. UPDATE: The application form was updated on Tuesday 20 September to address some minor faults. If you are having any difficulties filling in the form, please download th latest version. Ensure you have any supporting material you want to include. Question 5 of the application form invites you to include supporting material (maximum of five documents) which demonstrates your commitment to your chosen goal or area of interest. Get the medical referee form completed. This is on the last page of the application form. It can be completed by a GP, oncologist, care co-ordinator or social worker. Send us your completed application form and supporting material. How to nominate someone To nominate someone you'll need to fill in the application form (see above). Question 1 will need to be answered by the person you are nominating or scribed for them in their own words. You'll need their permission to nominate them and a good understanding of their personal history. Previous nominators have included parents, siblings, teachers, education professionals and advisers, social workers, care coordinators and oncologists.
